Connecting Health Professionals with Community Organizations
Stronger Together: Health and Community in LTCC is a collaborative event designed to strengthen connections between health professionals and community-based organizations that offer essential supports to individuals and families within the health sector.
Health care providers often support clients with complex needs that go beyond clinical care. This event creates space to learn about community programs, explore referral pathways, and build partnerships that enhance whole-person, community-centered support.
